Retainers are a type of orthodontic appliance that is worn over the teeth or bonded to them after the completion of fixed appliance therapy.
There are two types of retainers (fixed – wire and removable – plastic). This blog will focus on the second type, i.e., retention plastic retainers.
The purpose of using these almost invisible appliances is to maintain the results of the therapy, i.e., to help the teeth stay in their modified, corrected position.
How important are retainers in orthodontic therapy?
Orthodontic retention trays are made from plastic (medical polyurethane) and are strong enough for their intended purpose. At the same time, they are comfortable to wear, do not cause pain, and do not create significant discomfort for the patient.
They are recommended immediately after removing the fixed appliance as therapy to maintain the current appearance and position of the teeth.

The process of correcting irregularities in the upper and/or lower permanent dentition is long-lasting and often challenging. Therefore, the patient is expected to demonstrate great patience, persistence, and good hygiene habits. Along with regular dental check-ups.
Understanding the procedure itself and cooperating with the orthodontist is also very important.
Active movement of the teeth in most patients ends after two years. Once the appliance is removed, the retention phase follows – maintaining the achieved state.
For a while after completing orthodontic treatment, the teeth are unstable in their new position. Until the jawbones, alveoli, and ligaments adjust to the newly formed change.
Chewing or teeth grinding can lead to separation, shifting, and the teeth returning to their original position. Which could further lead to the development of malocclusion. This means that the teeth would no longer be aligned and perfectly straight.
Thus, if retainers are not worn during this sensitive period, relapse would occur, and all the effort would be in vain.
What is the additional role of retention plastic retainers?
In addition to establishing resistance of the teeth, retainers also help stabilize the bite.
During this period, it is important that the patient follows the instructions of the orthodontist regarding how to care for and how long to use the final phase of orthodontic therapy.
Are retainers the same for everyone?
No, retention retainers are made for each patient individually, according to the unique impression of their teeth, to which they fit. Just like fixed appliances.
An individualized approach is crucial even in the case of retainers.
Accordingly, patients may have the option to choose the color version of the retainers.
However, they are almost always transparent and are rarely made in any other color.

How long do you need to wear retainers?
The retention phase lasts at least 4–6 months. Retainers are worn 24/7, every day without exception, and it takes some time to get used to them.
Retention retainers are only removed when eating/drinking and for cleaning. They should not be chewed, especially not gum!
To prevent damage or breakage, they must be placed in a designated case.
They should never be left unprotected in a pocket or bag, or wrapped in a napkin on a table, as they can accidentally be thrown away.
In case of loss, new ones should be made as soon as possible to avoid unwanted teeth shifting.
Also, they should not be left in a hot car or near a stove or other heating appliances, nor within reach of pets that could chew them and cause permanent damage.
Hygiene of retainers
Hygiene is maintained the same way as brushing teeth: at least twice a day. Using a soft toothbrush, gentle movements, antibacterial solution, and warm (not hot!) water.
They should never be boiled, washed in a dishwasher, or placed in a microwave for any reason!
Once a week, they should be soaked in a special solution – a cleaner to remove food residues, bacteria, and plaque.
Toothpastes, which are naturally abrasive, are not recommended for cleaning retainers due to potential damage they could cause.
How does the age of the patient affect the use of retention aligners?
Since both children and adults can wear fixed appliances, retainers are a mandatory part of therapy for both groups.
However, the patient’s age can influence the duration of wearing retention retainers.
Namely, if a fixed appliance is worn by an adult, there is a higher probability that the teeth will return to their original position.
After the age of 18, the growth and development of the skeleton (and thus the jaw) is complete. And, the fibrous tissue connections to the teeth are stronger than in children. Therefore, in adult patients, the risk of relapse is higher.
Are retention trays necessary for you as well?
Based on the information provided, you can conclude for yourself: although fixed appliance therapy lasted a long time, you should not give up at the end. In this sense, retention retainers are also necessary for you.
